I state that I have read the Non-disclosure and Non-use Agreement and the Framework Contract that have to be concluded for the purpose of starting co-operation between me and Translation Street Jasiukiewicz-Szczurek sp. z o.o. with its registered seat in Poland, 30-363 Kraków, ul. Rydlówka 20; KRS 0000958693, VAT 6793172270, REGON 380859185, represented by Mr Stanisław Szczurek (hereinafter: “Translation Street”). I also state that I accept:
1. all provisions of these Agreements, as well as
2. the fact that the aforementioned Agreements will constitute the basis of co-operation between me and Translation Street until their termination by any of the parties in accordance with their provisions.
